The Intensive Care Department has 18 beds, including 14 inpatient beds and 4 emergency clinic beds that are always full of patients, most of whom need to closely monitor their vital signs.

Putting on a blouse, Dr. Thoa immediately got to work. She and another doctor are in charge of monitoring the health progress of about 18 patients in the department. They examine each patient room and then prescribe specific medications and procedures for each patient.

Taking advantage of the time to rest, Dr. Kim Thoa said: “The patient's recovery will continue continuously, requiring many intervention procedures such as: endotracheal tube placement, mechanical ventilation, central venous catheter placement, femoral vein catheterization and emergency dialysis… There are patients admitted to the hospital in a state of shock, it can take many hours to regain their lives. Sometimes they do not dare to be negligent, or even dare not Leave the patient for even a minute. Because the progression of some patients is very rapid, if not treated promptly, the patient may die.”
11:30 a.mthe clock also marked the end of the morning shift, Dr. Thoa and his colleagues temporarily put aside their work and ate a mid-shift meal.
While eating a bowl of rice, the bell from the emergency room rang continuously. Immediately, the entire crew put down their rice bowls and rushed to rescue the patient. After finishing work, when I came back, the rice and soup were cold, so I tried to swallow it to fill my stomach and prepare for a new shift.
13:30the female doctor and her colleagues continued with the work routine, the afternoon shift's workload was similar to the morning shift. At this time, the Emergency Resuscitation Department receives new emergency patients. Ms. Thoa and her colleagues in the department always examine and perform procedures for patients, then move some patients to other departments.

17:00at the time of overtime duty, Ms. Thoa and her crew were ready for a stressful night shift, which felt like “endless”. The normal night shift will be in the emergency clinic and in the Resuscitation Department. Every night we receive about 30-40 patients.
“Each patient usually takes up a lot of a doctor's time. The patient's progress is hourly, so the doctor on duty has to closely monitor him, with almost no rest. Faced with the fragility between life and death, requires brainstorming decisions in just a few minutes,” said Dr. Kim Thoa.
All night long, Dr. Thoa and his crew worked tirelessly without stopping. From receiving patients, with all kinds of different injuries or diseases. Go to the emergency room, examine this patient, then resolve the requests of the other patient's relatives. Just like that, the whole crew was busy with work, “taking a nap” seemed far away for them.
7am the next morninghanding over the shift, everyone's face was haggard, haggard, with dark circles around their eyes after a night of emergency duty, but no one complained and always told themselves that this was the profession they had chosen.
Having worked for 13 years at My Duc District General Hospital, Dr. Pham Thi Kim Thoa initially worked in the Department of Internal Medicine, but later fell in love with the Department of Intensive Care, now for nearly 12 years. During that time, more than anyone else, Dr. Thoa felt and understood the hardships of being a female emergency physician.
She confided: “Because I'm a woman, on duty days, my children are left unattended. There are times when I have to take my children to care before going on duty. Women working in the emergency resuscitation department are really very hard and strenuous. The work level is high, the pressure is high, and you love your job so much that you can stick with it for a long time. Many times, due to lack of human resources, doctors have to stay and work the morning shift after being on duty.”

It's so hard and tiring, sometimes you have to sacrifice your own health, you don't have time to raise your family and take care of your children. After 13 years of work, Dr. Thoa's income, including salary and allowances, is about more than 10 million VND per month.
“Combined with the husband's salary, the family's total income is 20 million VND. This amount of money, if spent sparingly, can only meet the minimum needs of the whole family such as: raising children, tuition, household expenses. family, not enough to save. To have more money to save for children in the future, or in case of illness or unforeseen circumstances, in addition to salary, medical staff are required to work outside. And of course If I work outside, I can't devote all my attention to the hospital,” the female doctor confided.
When asked if such an allowance is worth the effort she puts in, the female doctor frankly said: “The current allowance level is not satisfactory. With the current 24/7 allowance, it is 90,000 VND/person, really not commensurate with the labor of medical staff. For example, in a month, a doctor is on duty for about 8 days, receiving an allowance of less than 1 million VND This is compared to the effort of one person on duty 8 times a day, 24/24, which is clearly too low and inadequate. We do this for responsibility, professional ethics, for the patients, and even to mention the actual cost of being on duty no one wants to do it!”.
The hard work, modest income, and pressure from the family economy sometimes prompted her to quit her job at the hospital to work outside of the hope of having a decent income, to take care of a better family life. But with a great love for her job and the joy of saving patients' lives, she is still trying and contributing every day here.
Receiving a reporter's question about the proposal to increase on-duty allowances for medical staff, the female doctor excitedly replied: “If the on-duty allowance is increased, we medical staff will be very happy. Because when the allowance is increased, On-duty allowances will be more commensurate with the medical staff's efforts, our sweat and efforts will be recognized and appreciated more. Thus, medical staff will have more motivation wholeheartedly contribute to hospitals, especially public hospitals.”

Talking to reporters, doctor Tran Xuan Khuyen – Deputy Director of My Duc District General Hospital said he strongly agreed and supported the Ministry of Health's proposal to increase medical staff allowances. He added that currently, duty allowance levels are being applied according to Decision No. 73/2011. The fee for 24/24 hours at the district hospital is 90,000 VND/person. Over the past 13 years, the base salary has been adjusted 8 times, currently at 2,340,000 VND/month, while on-call, surgery, and procedure allowances have not had a corresponding adjustment.

“For the medical staff on duty at the Department of Intensive Care, Surgery, and Obstetrics, during the night they have to do a lot of work, very hard. With the current allowance level, it is clearly not commensurate with the effort they put in. “Furthermore, life has changed a lot, market prices of everything have increased over time, but the allowance level for medical staff remains the same, which is a disadvantage for the medical team,” Mr. Khuyen said. .
Mr. Khuyen added: “I very much support the proposal to increase allowances of the Ministry of Health. I really hope that the allowances of medical staff will be increased commensurate with their labor. To ensure the lives of our brothers and sisters, medical staff, thereby enhancing responsibility in serving patients.”
Master and Doctor Nguyen Tuan Thanh – Head of General Planning Department, Hong Ngoc General Hospital (Hanoi) said that medical staff at the hospital always comply with the provisions of labor law. Accordingly, a working day at the hospital is 8 hours, starting at 7:30 a.m. and ending at 5:00 p.m. During this period, medical staff enjoy full working regimes and lunch breaks.
Mr. Thanh added that currently the hospital's doctors and nurses have implemented a 24-hour standby regime. When participating in duty, the next day the employee will have 1 day off in accordance with regulations of the Ministry of Health.
In addition, the hospital also provides allowances for on-duty fees and meals for medical staff when on duty. In addition, according to the hospital's policy, there is also additional support for medical staff participating on duty according to a separate contract agreement between the two parties.
Speaking more about the proposal to increase allowances for medical staff, Mr. Thanh expressed: “As a medical staff, I am very happy, agree, and support the Ministry of Health's policy of increasing allowances. For the medical sector There is always a constant risk of exposure to disease, and there are unique pressures. Doctors and nurses also go through a long and intense training process. Furthermore, health care is a very specific industry The tolerance is very small. Therefore, increasing allowances for medical staff is urgent, it will create more motivation for them to work more effectively.”
Sharing the same opinion, Mr. Nguyen Hoang – Head of Human Resources Department, Hong Ngoc General Hospital expressed his full support for the proposal to increase allowances for medical staff.
“Medical staff are the ones who are always at the forefront of epidemics. Previously it was the COVID-19 epidemic, but now some areas are having measles epidemics, medical staff are the ones who always carries the responsibility of taking care of people's health. To ensure the lives of medical staff, the Ministry of Health's proposal is reasonable,” Mr. Hoang said.
